Sharing medication-related information with healthcare providers

Keeping track of and sharing medication-related information with your healthcare providers may seem overwhelming. Here are some strategies and key pieces of information to help make it easier.

During your appointment: explain your medical history, ask questions and discuss concerns, understand recommendations, and ask about next steps. After your appointment: talk to your provider before stopping medications, call your provider if you have concerns, make follow-up appoints if needed, and record additional questions for your next appointment.

Strategies for keeping track of medication-related information

  • Take (dictate or write) detailed notes in your phone (e.g. notes or app) or in a notebook; or record the entire visit using a voice note on your phone
  • Keep your notes up to date
  • Create a list of questions or concerns to discuss with your healthcare providers before your appointments
